Rx Square Medical Clinic can provide official medical documentation for workplace or school absences when a physician visit has determined that time off was medically warranted. Notes are issued by our physicians and carry the professional weight your employer or institution requires.
What to expect
To receive a medical note, you must first be assessed by one of our physicians during an in-person visit. Your physician will evaluate your condition and, where appropriate, provide written documentation confirming that you were seen and that medical leave was warranted for a specified period. Notes are not issued over the phone or without a consultation. Depending on your situation, same-day issuance may be possible.
Who this is for
Medical and sick notes are available to patients who have been seen at Rx Square for the relevant illness or condition. Please note that this service is not covered under Alberta Health Care and is subject to an uninsured services fee. Bring your AHCIP card and be prepared to pay the applicable fee at the time of your visit. Specific fee amounts are available by contacting the clinic directly.
